Morpho-Dynamics of the Body Furrows and Wrinkles
Aesthetics of the human body is a
balance of furrows and orifices, and artists of every period have studied the distance between the acoustic pavilion and the eye, eye and the nostrils, nostrils and the mouth, nipple and the mammary fold, sub-gluteus folds and iliac crest and so on to create a code for all body proportions.

Today, female beauty has more oblong shapes, compared to past models and surgical body modelling, performed in accordance with the anthropometric rules, aims at altering the orientation and depth of those aesthetic references points such as body folds and curves, by a combination of techniques.

These must integrate with the bio-dynamics of the human body, in order to prevent as well as correct defects and irregularities of the body, which has an inherent disposition to alter its shape, owing either to the physical forces influencing it, or the constant replacement of its tissues.

As the surgeon operates on an evolving matter, the quality of results will also depend on the number of adjustments and additional touches.The restoring of the body's symmetry and harmony are the goals that aesthetic plastic surgery pursues on a constantly evolving subject, such as the human body.

Ideal proportions of the female body, instruments for surgical body modelling, widening of physiognomic points after fat graft, techniques for undermining, suctioning and modelling the subcutaneous layer are described in this work.


L'estetica del corpo umano è un giusto rapporto tra solchi e orifizi, e gli artisti di tutte le epoche hanno studiato la distanza tra meato acustico e occhio, tra questo e le narici, tra le narici e la bocca, tra capezzolo e solco mammario, tra solco gluteo e cresta iliaca etc., per codificare le proporzioni corporee.

Oggi, la bellezza femminile ha forme più oblunghe rispetto al passato, e il modellamento chirurgico del corpo, compiuto in conformità con i canoni antropometrici, mira a cambiare l'assetto di quei riferimenti estetici e morfodinamici che sono le pieghe, i solchi e le curve del corpo con tecniche combinate.

Queste debbono integrarsi nella bio-dinamica del corpo umano, allo scopo di prevenire, oltre che correggere, i difetti e le irregolarità della forma corporea, che è in costante trasformazione in virtù sia delle forze fisiche che agiscono su di essa, che del continuo ricambio dei costituenti tessutali.Sono qui descritte le tecniche di antropo-cefalometria, gli strumenti e le metodiche chirurgiche per il modellamento corporeo.

Brow ptosis is the main consequence of the aging of the upper third of the face. Many methods have be en described to correct it: skin excisions of the brow hairline, skin excisions of the forehead natural creases, skin trimmings of the temporal, coronal or forehead hairline flaps, and endoscopic methods [1,15,17,18,24,26,27].
The authors created a procedure which is based on a forehead-temporal sub-cutaneous flap and a muscular relocation. It treats the brow ptosis and its surrounding area : the temporal ptosis, the upper and lower lateral eyelid ptosis, the crow's feet, and at the same time improves the sclera show or ectropion. The method preserves the sideburn and the temporal hairline and can reduce the width between the temporal hairline and the lateral end of the eyebrow. The method produces maximum improvement, with high-quality scars and minimal evidence of surgery.
The procedure is called "The Toucan Technique", due to the shape of the skin resection which looks like a toucan bird. [11,13,14].



Subglandular versus Subpectoral pocket for breast prostheses has been a subject of discussion over the past quarter century.
In 1994, in order to increase the volume, enhance the shape, and improve the breast projection, the author idealized a procedure that took advantage of the virtual spaces found in the breast anatomy, used simultaneously the subglandular and subpectoral areas by locating the prostheses in a double pocket on both sides of the pectoral muscle, joined through a gentle muscle buttonhole.

La Chirurgia Maxillo-Facciale, demolitiva, ricostruttiva, od estetica ha da sempre avuto bisogno di materiali che permettessero la sintesi , la sostituzione o l' integrazione di elementi anatomici delle ossa mascellari e dello scheletro facciale.Lo sviluppo della bio ingegneria di pari passo con la miglioria delle tecniche chirurgiche ha portato all'utilizzo quotidiano di materiali ormai indispensabili per la pratica chirurgica.
Vengono qui elencati e classificati i biomateriali indicando l'utilizzazione clinica di quelli che in Chirurgia Maxillo-Facciale sono routinariamente impiegati.
Metalli, materiali ceramici o polimeri di sintesi non agiscono attraverso meccanismo chimico e non hanno bisogno di essere metabolizzati ma è grazie alla loro biocampatibilità che permettono di ricostruire o di sostituire i tessuti duri dello scheletro facciale ripristinando la funzionalità e l' eumorfismo del volto.

La tossina botulinica, inizialmente utilizzata in campo oftalmologico, trova oggi largo impiego in medicina estetica, in particolare negli USA dove è diventato fenomeno di massa, nel trattamento delle rughe dinamiche facciali, dette anche "rughe di espressione".
L'uso più diffuso in questo campo riguarda le rughe del terzo superiore del viso: frontali, glabellari e perioculari. Negli ultimi tempi si va sempre più diffondendo l'uso per l'innalzamento del sopracciglio, la distensione delle rughe del collo (collana di venere) e l'iperidrosi ascellare e palmare.
Esistono alcune controindicazione all'uso della tossina, in particolare nei pazienti affetti da patologie neuromuscolari o che assumono antibiotici aminoglucocidi, anticoagulanti o prodotti che interferiscono con la trasmissione meuromuscolare. Anche lo stato di gravidanza o di allattamento sconsiglia questo genere di terapie.
Le tecniche di utilizzo e le dosi da inoculare variano in funzione dell'inestetismo da trattare, della tipologia di prodotto utilizzato e della massa muscolare interessata.
Il trattamento è di tipo reversibile e la durata dell'effetto dipende da quanto il paziente solleciti, attraverso la mimica, la zona trattata. Risultati insoddisfacenti possono derivare dalla inadeguata conservazione o ricostituzione del prodotto, dalla presenza di anticorpi neutralizzanti da parte del paziente o da fattori psicologici. Anche se raramente, si possono presentare effetti collaterali indesiderati quali: paralisi temporanee dei muscoli limitrofi alla zona di inoculazione, ecchimosi o cefalee.

Tra gli innumerevoli interventi proposti dalla chirurgia e dalla medicina estetica certamente l'autotrapianto di capelli occupa un posto di rilievo per recuperare sicurezza e piacevolezza estetica e per riportare indietro l'inesorabile orologio del tempo. La calvizie comune è quella frequentissima affezione del cuoio capelluto che, causando un'alterazione del profilo estetico dell'individuo, determina problemi psicologici e relazionali ed interferisce negativamente con la qualità di vita. Le tecniche chirurgiche per la cura della calvizie sono numerose e possono essere usate singolarmente od in associazione per ottenere il miglior risultato possibile. Da circa dieci anni, con l'avvento della tecnica dei micro e mini innesti, l'autotrapianto rappresenta la scelta d'elezione per il trattamento dell'alopecia androgenetica. Premesso che l'intervento presuppone un'esperienza specifica nel settore ed un'équipe numerosa e preparata, il chirurgo che si appresta ad un intervento di autotrapianto deve essere al corrente delle regole che permettono di ottenere un risultato estetico ineccepibile. Infatti non è sufficiente utilizzare innesti mono o bibulbari, bisogna che gli stessi siano prodotti di qualità, che siano trapiantati rispettando la direzione di emergenza del capello ma soprattutto bisogna esser in possesso di senso artistico e gusto estetico unito alla razionalità di progettazione. Mimare la natura non è facile ma è la sola possibilità che abbiamo per eseguire un intervento soddisfacente per il paziente e per l'operatore. In questa comunicazione sono espressi i criteri fondamentali e le regole per poter affrontare con successo questo intervento che solo apparentemente può apparire semplice e di facile realizzazione.

Le cellule eucarioti dopo un numero finito di divisioni cellulari entrano in uno stato di quiescenza caratterizzato dall'arresto della crescita e dall'alterazione di alcune funzioni fisiologiche. La teoria più accreditata considera che lo stato di senescenza sia un processo, controllato geneticamente, che costituisce un meccanismo di soppressione di una eventuale crescita neoplastica. Le moderne tecniche di indagine del genoma (microarray, proteomica) potrebbero, dunque, condurre all'individuazione dei geni che presidiano il processo di invecchiamento e alla delucidazione di quali siano le reali cause di un evento che accomuna tutti gli esseri viventi. A tutt'oggi, ciò che riusciamo ad individuare sono solo gli effetti, alcuni precoci, altri più tardivi, del processo. La comprensione di alcuni di essi, tuttavia, ha consentito di avviare una intensa ricerca su molecole che possano contrastare o, quanto meno, rallentare il processo di invecchiamento. L'accumulo di radicali liberi, ad esempio, è stato uno dei primi eventi ad essere correlato con l'invecchiamento cellulare e una profusione di molecole con capacità antiossidanti sono allo studio o già ampiamente presenti sul mercato. Una teoria più sofisticata e innovativa, in qualche modo collegata alla produzione e accumulo delle specie reattive dell'ossigeno (ROS), considera invece un possibile coinvolgimento dei mitocondri, gli organelli cellulari deputati alla produzione di energia, nel processo di invecchiamento e demanda al DNA mitocondriale, un DNA ancestrale tramandato di madre in figlia, il ruolo principale nel controllo genetico del processo di senescenza. La produzione di energia nei mitocondri è assicurata da sistemi multienzimatici che supportano le reazioni chimiche di ossido-riduzione del metabolismo del glucosio. L'acido lipoico, una molecola strutturalmente molto semplice e funzionalmente molto complessa, è il composto-chiave a cui si deve l'innesco dell'intero meccanismo di produzione dell'energia cellulare. Anche gli ormoni e altre molecole coinvolte nella trasduzione cellulare del segnale, la complessa rete attraverso cui la cellula trasmette i messaggi al nucleo per controllare l'espressione dei geni e la produzione di proteine, giocano un ruolo nella senescenza. Alla terapia ormono-sostitutiva, oggetto oggi di un dibattito infuocato sugli effetti indesiderati ad essa correlati, si affiancano molecole con attività ormono-simile e reperite dal mondo vegetale, i cosiddetti fitoestrogeni. Flavonoidi e lignani abbondano in alimenti tipicamente presenti nella dieta orientale (soia e derivati, the) e mediterranea (leguminose, crucifere). Alcuni di questi principi attivi, purificati, sono allo studio per il controllo di quei meccanismi che determinano i processi degenerativi.


Eukaryotic cells, after proceeding through a finite number of cell divisions, enter a state characterized by irreversible growth arrest and altered function. The most accomplished theory describes senescence as a genetically controlled process that constitutes a tumor suppressive mechanism. The innovative techniques to investigate the human genome (microarray, proteomics) may lead to recognize a number of genes involved in aging and to highlight the causes of such an event, common to all living things. What scientists have described until now represents only the effects of the process, early or late steps of a more complex mechanism, whose comprehension, however, triggered studies on those molecules able to block senescence or, at least, slow it down.
Production and cell accumulation of free radicals is a well-known event related to aging. A lot of antioxidants are under investigation and many entered the market as well.
A more suggestive and innovative hypothesis, a sort of a variant of the reactive oxygen species (ROS) theory, relates mitochondria, the source of energy in the cells, to aging. Mitochondrial DNA was proposed to play the major role in aging.
The mitochondrial energy production is supported by complex multienzyme systems, involved in redox chemical reactions, which glucose is metabolised through. Lipoic acid, whose simple chemical structure contrasts with its intricate action, plays the key role.
Hormones and other molecules involved in cell signal transduction, the complex network that cells use to transfer messages to nucleus in order to control gene expression and protein production, should be considered in senescence process. The replacing hormone therapy, whose risks and benefits are being debated, has been joined by phytoestrogens, hormone-like molecules from plants. Flavonoids and lignans are present in Far East diet (soya and soya derivatives, the) as well as in Mediterranean diet (leguminosae, brassicaceae). Some of the active compounds have been purified and are now investigated to assess their role in controlling the mechanisms involved in cell degenerative processes.

Regeneration in the Nervous Sistem

Since 1981, regeneration in the nervous system is a matter of research in our Institute. In animal models we studied regeneration into the spinal cord and peripheral and cranial nerves.
It is well known that axonal regeneration occurs in peripheral nerves but not in the spinal cord. Factors like the oligodendrocitic origin of the myelin, the lack of growth factors or the presence of inhibitory factors should inhibit axonal regeneration in the central nervous system. Therefore, we tried to modify the central nervous system environment by using transplants or providing growth factors. In our first rat model, the thoracic spinal cord was transected and two peripheral nerve autografts were put as bridges to by-pass a transverse injury. These peripheral nerve grafts were colonized by regenerated axons that enter the graft, elongate along it and re-enter for a few millimeters into the spinal cord. Neurons of the spinal cord and dorsal root ganglia gave origin to these regenerated axons. Local supply of nerve growth factor (NGF) significantly enhanced such regenerative responses.
In another experiment, we transplanted segments of thoracic embryonic spinal cord into the adult rat thoracic spinal cord at the level of a transverse section. Embryonic transplants survived connecting the spinal cord stumps. Axons from neurons of the transplant enter and shortly advance into both spinal cord stumps, as well as, axons of neurons of both spinal cord stumps shortly enter the embryonic transplant without crossing it completely. Subsequently, spinal cord regeneration was limited to study one single long tract, the corticospinal one, establishing its time course of degeneration after spinal cord transection. Then, we tried to influence that phenomenon by local administration of NGF and vascular endothelial growth factor (VEGF). VEGF was administered with a gene therapy approach using an adenovirus carrying the gene for the VEGF. Both NGF and VEGF inhibit degeneration of the corticospinal tract axons; VEGF promoted also axonal regeneration across the injury site. Recently, we are using transplants of neural stem cells of the adult mouse into the injured spinal cord of mice. The main problem was to recognize stem cells into the host spinal cord after cell division, migration and differentiation. We have overcome this problem introducing into the stem cells a retrovirus carrying the gene for a green fluorescent protein. The problem of spinal cord regeneration is unsolved and further studies are necessary.
In peripheral and cranial nerve regeneration, we studied the changes occurring at the motor nuclei within the brain stem or the spinal cord after section and repair of the oculomotor, facial and sciatic nerve. The original unilateral subnucleus or nucleus became bilateral and motoneurons increased in number. By studying the changes of one subnucleus of the facial nucleus, the stylohyoid muscle subnucleus, after section and repair of the facial nerve, it was observed that the original unilateral subnucleus became bilateral, the number of motoneurons increased and several motoneurons were located outside the normal limits of the stylohyoid subnucleus, even outside the limits of the entire facial nucleus. The ectopic motoneurons were seen along the way followed by cells during the embryogenesis from the floor of the fourth ventricle to the ventral part of the pons to form the facial nucleus. On this basis, the increased number of motoneurons was hypothesized as new neuron formation from stem cells located in the subependymal layer of the fourth ventricle of adult rats. Nerve repair should trigger a regenerative response that involves the ipsilateral nucleus, the homologous contralateral nucleus, and the regional stem cells. A certain repetition of embryonic phenomena could occur at the nuclear level after nerve repair.
In another experiment, after hypoglossal-facial anastomosis not only hypoglossal motoneurons reinnervate facial muscles, as it was expected, but surprisingly also facial motoneurons send axons to facial muscles through the hypoglossus. More recently, after repairing the third cranial nerve with the facial nerve, the oculomotor muscles were re-innervated by motoneurons of the facial nucleus of both sides, of the abducens and motor trigeminal nuclei of the same side and of the trochlear nucleus of both sides.
After section of peripheral and cranial nerves, degenerative changes occur in both the sensory and motor system. The retrogradely degenerative events initiate at the first neuron station to spread to all the stations of the neural circuit up to the motor and sensory cortex. Conversely, after nerve repair regenerative phenomena occur in both the sensory and motor system. We have studied the regenerative events occurring at the first motor station. A phenomenon occurring peripherally, like a nerve repair, may trigger diffuse central regenerative responses. The already demonstrated existence of stem cells in the central nervous system of adult mammals may help to explain the increase in number of motoneurons observed after nerve regeneration. Peripheral nerve regeneration has a self-protecting meaning and could be associated with the self-protecting function of stem cells which seem to be attracted by injured areas, like a motornucleus is after nerve section and repair. Our further studies aim to demonstrate that nerve regeneration is not only axonal regeneration but also generation of new neurons.

After the demonstration that spinal cord stimulation (SCS) can improve peripheral blood flow, Hosobuchi ('86) first studied the effect of SCS on cerebral blood flow (CBF) in human beings. Our group found that SCS can produce either an increase of CBF or a reduction or no effect. In patients studied with both SPECT technique and TCD the sign of the induced variations, when present in both, was the same. A reduction of CBF is very rare and occurs for more caudal electrodes location while cervical stimulation produces more frequently an increase in CBF (61% of cervical stimulations). Autoregulatory tests performed in our laboratory in SCS patients before and during stimulation suggested that SCS and CO2 interact with the mechanism of regulation of CBF in a competitive way. We confirmed the SCS effect on CBF in rabbits suggesting that a reversible functional sympathectomy occurs during SCS.
Beside experimental reports suggesting that SCS 1) drastically prevents cerebral infarction progression in cats, (Matsui '89); 2) improves clinical symptoms of patients in persistent vegetative states (Kanno '89); 3) suppress headache attacks in migraneous patients (Robaina '90); 4) significantly reduces ischemic brain oedema in rats (Gonzalez-Darder '91), Hosobuchi first suggested cervical SCS for the treatment of "intractable"cerebral ischaemia in man ('91).
After the demonstration that SCS prevents experimental "early vasospasm" (Acta Neurochir 2001; 143:177-185; Stereotact Funct Neurosurg 2001; 269-275) and experimental combined ischaemic - traumatic damage in rabbits (Stereotact Funct Neurosurg 2001; 76: 276-281), our group studied the effect of SCS on and stroke in humans Sterotact Funct Neurosurg 2001; 76: 262-268).
So far we have successfully implanted 18 patients harbouring ischaemic brain damage for different indications (spasticity, chronic pain, bladder dysfunction) ; 16 out of 18 patients presented changes in CBF (as measured as with TCD, SPECT and Near Infrared Spectroscopy), (variable in signs) during SCS. Such a phenomenon could be identified as a "functional" neuroplasticity. As matter of fact the evidence of a redistribution of CBF more than a clear increase is shown in such patients as well, raising the question on the supposed role of ischaemic penumbra in mediating clinical improvement; up to date no clear indication exists on SCS in stroke patients without spasticity, chronic pain and bladder dysfunction.

La moderna neurobiologia ha prodotto dati fondamentali che ci hanno consentito di considerare il CERVELLO non come un insieme di neuroni predeterminati, ma come un continuo dinamismo di interazioni cellulari,sia nel corso dello sviluppo sia nell'individuo adulto, in cui i fenomeni di plasticità neuronale e le modificazioni delle mappe corticali contribuiscono alla espressione biologica della individualità.
Gli studi attuali in Neuroscienze hanno dimostrato che il sistema nervoso centrale (SNC) è notevolmente plastico nella prima infanzia dell'animale e dell'Uomo e che tale proprietà è mantenuta , anche se in minor misura, nell'adulto. Il motore della plasticità è dato dall'esperienza sensoriale su una base di predisposizione genetica , che è diversa da animale a animale. Per ogni animale ed anche per l'Uomo si definisce un periodo in cui il SNC risponde con cambiamenti a livello della sua struttura anatomica e fisiologica , in maniera particolarmente efficace. Tale lasso temporale è definito - " periodo critico"- nell'Uomo è dell'ordine di alcuni anni.
Si ritiene che lo sviluppo del cervello sia in parte guidato dai geni e in parte dall'esperienza.
Lo sviluppo dipendente dai geni non consente libertà di intervento all'individuo poiché geneticamente codificato e programmato mentre con la nascita e quindi l'esperienza sensoriale, si apre la libertà individuale di sviluppare, arricchire e potenziare la propria struttura nervosa sempre, tuttavia, nei limiti consentiti dai geni.
Si consideri la modalità sensoriale "visione" (area 17 della corteccia visiva) :
qualora si instauri un deficit prolungato riguardante l'esperienza visiva durante il "periodo critico" in un occhio la visione sarà permanentemente danneggiata perché la struttura delle colonne di dominanza oculare deputate a quest'occhio subisce processi regressivi di riduzione mentre quelle deputate all'occhio non deficitario si espandono . Questo affascinante fenomeno dell'esperienza ci suggerisce che nel cervello dei mammiferi esista una"grammatica molecolare" dell'apprendimento e che le mappe somatotopiche cerebrali possano essere modificate dall'esperienza.

Nella relazione si pone l'accento su come l'attività intensa aumenti fino a 50 volte la produzione biochimica di radicali liberi. Ma se nell'atleta esistono dei meccanismi di compensazione a questa over production, nel soggetto non atleta la ricerca della fitness a tutti i costi può portare a danni cellulari con l'insorgenza di gravi malattie ed invecchiamento precoce, portando, pertanto, a risultati nocivi ed "opposti" rispetto a quelli ricercat.
Pertanto, la omeostasi e una buon contrasto all'invecchiamento si dovrà ricercare con una giusto equilibrio fra dieta anti radicali e fitness controllata.

I sistemi a moltissime particelle mostrano proprietà fisiche non riconducibili a quelle delle singole parti. La scoperta delle nuove proprietà è un campo di ricerca molto interessante sia dal punto di vista speculativo che tecnologico. Sono descritti, ad illustrazione della tesi, alcuni semplici sistemi che mostrano proprietà fisiche collettive e si discute come, rendendo più complesso il sistema, proprietà collettive diverse possono dare origine a nuovi tipi di proprietà collettive.
Inoltre lo stesso sistema fisico può presentarsi sotto forme diverse (per esempio geometriche, ma non solo) che danno luogo a proprietà collettive differenti e che, a loro volta, per aumentata complessità del sistema, determinano nuovi tipi di proprietà collettive.

Il progressivo rilievo assunto dalle problematiche giuridiche e medico-legali relative alla responsabilità professionale medica ha naturalmente coinvolto anche l'attività dello specialista in chirurgia estetica soprattutto in ordine alle maggiori aspettative del paziente riguardo a questo tipo di trattamenti.
Dopo una sintetica disamina di tali problematiche in generale gli Autori affrontano il problema specifico della responsabilità del chirurgo estetico ribadendo soprattutto l'equivalenza dei criteri del giudizio di responsabilità in tale settore specialistico rispetto alle altre branche medico-chirurgiche, sia per quanto riguarda l'oggetto di obbligazione (di mezzi e non di risultato) sia per quanto riguarda il dovere di una valida informazione del paziente.

Al fine di fornire una concreta esemplificazione delle problematiche inerenti la responsabilità del chirurgo estetico vengono illustrati alcuni dei casi più significativi di una casistica personale, relativi a complicanze conseguite rispettivamente ad un intervento di liposuzione e ad un trattamento crioterapico per iperpigmentazione cutanea del volto.
